Access Cards
Each household can receive up to 2 free access cards.
Any additional card or loss of card will be $15.
Waviers
Must Be Renewed Annually by ALL Residents
Every person, or a parent/legal guardian of a person under the age of 18, desiring to use or participate in activities on/in/ around a St. Vrain Lakes Metropolitan District No. 1 (the "District") recreational amenity including, but not limited to swimming facilities, the Cove, fitness center, playgrounds, lakes, parks and trails (each a "Facility" and together the "Facilities") must complete this Waiver of Liability and Release Form.

Forms
User Age and Use Restrictions - Swimming Pool. No person under the age of 18 may access the pool unless
accompanied by a parent, legal guardian or other adult aged 18 or older as authorized on
the User Information Form. Persons between the ages of 13 and 17 are permitted to access
the pool without adult supervision provided such person has a current and signed Minor
Release Form on file. Persons aged 12 and under are permitted to be accompanied by a
minor over the age of 16 as authorized by the parent or guardian on the Minor Release
Form.
User Age and Use Restrictions - The Cove. No person aged 18 or under may use The Cove unless
accompanied by a parent, legal guardian or other adult aged 18 or older as authorized on
the User Information Form. Persons between the ages of 13 and 17 are permitted to access
The Cove without adult supervision provided such person has a current and signed Minor
Release Form on file. Persons aged 12 and under are permitted to be accompanied by a
minor over the age of 16 as authorized by the parent or guardian on the Minor Release
Form.
Additional Authorized Users: Includes: Adult Children (18+); Tenants/Renters; Regularly Employed Caregivers (Nannies/Babysitters); Immediate Family Residing at Property Address (incl. Grandparents/Grandchildren); and/or others residing at your property address (PROOF MAY BE REQUIRED).
Additional Authorized Users Do NOT Include Guests. Additional guest passes are available and/or may be purchased at the Cove fitness Center during operational business hours. Please refer to SVLMDs Amenities Fee Schedule for additional information.
ALL ADULT ADDITIONAL AUTHORIZED USERS MUST COMPLETE AN ADDITIONAL AUTHORIZED USER FORM AND BE LISTED ON THIS PROPERTY OWNER FORM IN ORDER TO ACCESS THE RECREATION AMENITIES.
